A one-of-a-kind risk management program developed with the exclusive support of Lancaster General Hospital and Ephrata Community Hospital, PHC works to assist members by giving them a better understanding of their own health and allowing them to take greater ownership in their personal well-being. Managed by PHC's Medical Director, Preferred Wellness provides employers and members access to the consultative services of a nationally respected expert in medical management, disease management and medical informatics. Preferred Wellness helps to identify current risks and risk of future health problems and their associated high costs; assist people to become ready to change; and increase the likelihood of a net positive change through care coordination and support by the patient's primary care giver and PHC's clinical support staff.
